Your Contact at LTCS

Your LTCS coordinator is your main contact for the Lifetime Care and Support after you are accepted into the Scheme.

Their job is to work with case managers and service providers to help you get the support that's available through the Scheme. When you've been in the Scheme for a longer time or when your needs are stable, you may no longer need a case manager to help you. In these circumstances you can request services directly with your LTCS coordinator. They will:

• make contact with you and discuss our services and the support we provide
• provide information about eligibility and how to apply
• help coordinate injury-related services (like rehabilitation or attendant care)
• help plan any return to school, leisure activities or work.
